Sponsorship Form

Before you fill out the below form please have the following ready:
  • If paying by Credit Card on the Bike MS website (PREFERRED METHOD), have the Credit Card ready.
  • If paying by Check, please have check handy so you can fill in the Check # in this form. All checks should be made out to "National Multiple Sclerosis Society."
  • If donating a product/service for the team or fundraiser please have all details of the item(s) and their value.
Sponsorship Process:
  • STEP 1
    • Fill out the below form. We must receive a completed online form for your sponsorship to be considered official–thanks for understanding.
    • You will receive a copy of your Sponsorship commitment to the contact email listed in the form.
  • STEP 2
    • The emailed copy of your Sponsorship commitment will include a link to pay with credit card directly on the Bike MS website or directions on addressing/mailing your check.
    • If you are a first time sponsor OR your logo file has changed from last year, you will receive a second email with instructions on submitting your logo file.
